Section 166.3 - Applications(a) Each eligible agency applying for a grant under this Part shall submit an application on forms provided by the department. Such applications shall be submitted not later than June 1st of each year for programs to be conducted during the following school year, provided that applications for grants for the 1987-88 school year shall be submitted by a date to be prescribed by the commissioner. The commissioner may grant a variance from this time limit under extraordinary circumstances.(b) The application shall set forth the following information with respect to each program for which a grant is requested: (1) a breakdown of the number of workers proposed to be served, including: (i) the number of workers with inadequate literacy skills;(ii) the number of workers whose present levels of literacy skills constitute a barrier to continued employment;(iii) the number of workers whose present levels of literacy skills constitute a barrier to promotion;(iv) the number of workers in threatened industries;(v) the number of workers in jobs undergoing technological change; and(vi) the number of workers at risk of being displaced;(2) a description of any existing adult basic education program operated by the eligible agency or by other organizations at the eligible agency's facilities;(3) a description of the proposed program, including the qualifications of the full- or part-time professional and auxiliary staff, the facilities to be used, the equipment and materials to be provided, the manner in which the program is to be administered and supervised, the number of hours of classroom instruction to be provided, and the planned enrollment of students in the program;(4) the proposed budget for the program, indicating in detail the manner in which funds are expected to be received and disbursed, including all funds used in connection with the workplace literacy and basic skills education program whether appropriated by the State Legislature to carry out the purposes of this Part, or otherwise appropriated or provided by a local government or public agency, local school district, private persons or organizations, or the eligible agency itself;(5) a copy of each curriculum to be used in the proposed program which describes the job-related literacy outcomes of the program; and(6) any other information required by the department relative to the planning, administration, funding or evaluation of the program.N.Y.
