Section 57-2.4 - Application for approval of provider

(a) Providers seeking approval pursuant to this Subpart shall submit to the commissioner an application on forms prescribed by the commissioner and a fee of $600 for the review of such providers.

(b) The commissioner shall review the information contained in such application and may request and review additional information and may conduct a site visit to ensure compliance with the requirements of this Subpart.
