The above-named defendant having been found not responsible by reason of mental disease or defect,
And, this court at the initial hearing conducted pursuant to subdivision 6 of C PL 330.20 having found that the above-named defendant is mentally ill but does not have a dangerous mental disorder, and this court having issued on this date a civil order committing the above-named defendant to the custody of the State Commissioner of Mental Health for care and treatment pursuant to the applicable provisions of the Mental Hygiene Law,
And, this court at the initial hearing conducted pursuant to subdivision 6 of C PL 330.20 having found that the above-named defendant does not have a dangerous mental disorder and is not mentally ill, and this court having issued on this date an order, pursuant to the provisions of subdivision 7 of C PL 330.20, discharging the above-named defendant subject to this order of conditions,
And, this court having issued on this date a transfer order pursuant to the provisions of subdivision 11 of C PL 330.20,
And, this court having issued on this date a release order pursuant to the provisions of subdivision 12 of C PL 330.20,
NOW, THEREFORE, it is
ORDERED that the above-named defendant comply with each term and condition of the treatment plan prescribed for the said defendant by the State Office of Mental Health, and it is further
ORDERED that whenever the above-named defendant is in the custody of the Commissioner of Mental Health and confined to a facility under the jurisdiction of the said Commissioner or in a facility designated by the said Commissioner, the said defendant shall not leave that facility without authorization, and it is further
ORDERED that if this order is issued in conjunction with a release order issued pursuant to subdivision 12 of C PL 330.20, the above-named defendant shall comply with each term and condition prescribed in the written service plan annexed to this order and made a part hereof, and it is further
ORDERED that the above-named defendant shall comply with the following condition or conditions: [Here insert any other condition which the court determines to be reasonably necessary or appropriate] ____________
