Section 473.4 - Changes in application
(a) Any change relating to the information contained in the application blank must be reported immediately to the State Tax Commission, Miscellaneous Tax Bureau. Thus, any change in the applicant's business or other address, or in the name or ownership of its business must be so reported.
(b) In addition, if a motor vehicle for which a permit has been issued is transferred or junked or if the business of a permit holder is sold, the commission should be notified promptly. In such cases, any permits, plates and stickers, issued to the carrier must be surrendered within five days thereafter. (See section 474.6, infra.)
(c) All correspondence, including blank tax return forms, from the commission will be mailed to the business address shown on the permit application unless the applicant notifies the commission of a change of address.
