Section 156.4 - Assumed names
(a) An assumed name is a name, other than its real name or fictitious name, under which an organization conducts business in this State. An assumed name is commonly referred to as a d/b/a.
(b) Form of certificates. The certificates that may be filed concerning assumed names shall be the Certificate of Assumed Name, which shall pertain to the initial adoption of such a name; Certificate of Amendment of Assumed Name, which shall pertain to changes in the information contained in the original certificate filed; and Certificate of Discontinuance of Assumed Name, which shall pertain to the termination of the use of the assumed name.
(c) Terms indicating form.
(1) An assumed name shall contain no indicator of organizational form (e.g., corporation, limited partnership, limited liability company, or the respective abbreviation).
