Section 459.6 - Other requirements(a) Record. Each provider of services shall obtain and maintain a record of the following information for each individual applying for and/or receiving services: (2) social security number;(8) minor children (if in home);(9) identity and address of person to be notified in case of emergency; and(10) date of registration.(b) Daily log. The senior citizen center shall maintain a daily record of individuals receiving services. Such daily log shall be centrally located so that participants can sign in each day they attend. The date and signature of the participant will suffice for minimum data requirements for this record. These logs shall be maintained by the senior citizen center for post audit by the social services official.(c) Participant record, DSS-1875. Senior citizen centers shall be required to complete and maintain a DSS-1875, participant record, or an equivalent approved by the State Department of Social Services, for each participant in their program. Only one record for each participant is needed. It will not be necessary to prepare these forms for all enrollees, since a record can be completed as they participate.(d) Assurances. The social services official shall obtain from a provider of services (P.O.S.), or submit to the department as a provider of such services, written assurances on an annual basis that the services provided meet the administrative and program requirements specified in this Part.N.Y.
