Section 347.18 - Records and reports
(a) The child support enforcement unit must establish a CSMS case record within 20 calendar days of receiving a referral under section 347.3 of this Part, and within 20 calendar days of receiving an application for child support services under section 347.17 of this Part. Based on an assessment of a case, the child support enforcement unit must, within the same 20 calendar days, as appropriate:
(1) solicit necessary and relevant information from the custodial parent and other relevant sources, and initiate verification of such information; and
(2) request additional information or refer the case for further location attempts, as specified in section 347.7 of this Part.
(b) The case record must contain all relevant information developed and obtained including, but not limited to, the following:
(1) a record of all contact with the applicant or recipient, the noncustodial parent/ alleged parent/intended parent, and the reason for such contact;
(2) a record of all efforts expended to locate the noncustodial parent/ alleged parent/intended parent, and the results of such efforts;
(3) a record of all efforts expended to collect, monitor and enforce support payments; and
(4) noncustodial parent/ alleged parent/intended parent identifying data, including name, last known address, date of birth, social security number, employer, wage and health insurance information.
(c) Each social services district must submit to the department monthly reports, required by the department on forms supplied by the department, which are due on or before the 20th day of the month following each report month. Such reports must include but not be limited to:
(1) a summary of collections and distributions;
(2) a distribution of administrative costs for title IV-D;
(3) an A-87 distribution of costs for title IV-D; and
(4) a statistical report.
(d) Each child support enforcement unit must keep such records as the DCSS may from time to time require.
