Section 2.5 - Proof of identity
(a) When records are to be made available in person following a request made either in person or by mail, the department may require appropriate identification such as a driver's license, an identifier assigned by the department, photographic identification, or some similar means to confirm the identity of the data subject.
(b) When a request is made by mail, the department may require that it be acknowledged by a notary public or other officer, or that an identifier generally known by data subject be included, or the department may require other similar appropriate identification.
(c) Proof of identity shall not be required for a record accessible pursuant to the Freedom of Information Law.
