Section 155.2 - Fees; procedures in appeals involving no hearings

(a) A fee of $10 which shall not be refundable must be paid at the time the appeal is filed from a determination not based upon a hearing.

(b) Appeals from a determination not based upon a hearing at which testimony was taken may be submitted on a form and in the manner prescribed for such purpose not later than 60 days from the date after written notice was given of the determination appealed from, excluding the date of the determination. A copy of the written determination from which the appeal is taken must be filed with the appeal form.
