N.Y. Comp. Codes R. & Regs. tit. 15 § 72.5
A registrant who wishes to participate in the Watch Your Car program may obtain a consent form approved by the Commissioner of the Division of Criminal Justice Services at any participating law enforcement agency throughout the State. The registrant must sign the consent form and file it with the participating law enforcement agency which provided the form. The consent form shall contain information identifying the registrant and the vehicle and authorized users; a statement that the vehicle is not normally operated between the hours of 1 a.m. and 5 a.m., or, if the registrant has chosen to utilize the international decal, at any time in the proximity of a port or an international border, and the registrant's consent to a stop by a police officer if the vehicle is operated by anyone at such time or location; instruction on removal of the decals upon withdrawal from the program or transfer or disposal of the vehicle and such other information as the Commissioner of the Division of Criminal Justice Services may prescribe.
