Bills for therapy treatment or services shall be submitted by a self-employed therapist to the employer or carrier together with a copy of the prescription or referral by the authorized physician or in the case of physical therapy, authorized physician or authorized podiatrist, including directions as to treatment, if any. Controversies as to the reasonable value of such charges shall be determined by an arbitration committee in accordance with section 13-g of the Workers' Compensation Law and the rules thereunder.
