Treatment shall be rendered by a duly licensed therapist, upon prescription or referral which may be directive as to treatment of an authorized physician or in the case of physical therapy, authorized physician or authorized podiatrist. Treatment by a licensed therapist shall be within the scope of such therapist's specialized training and qualifications as defined in articles 136 and 156 of the Education Law. Prescription or referral by the authorized physician or in the case of physical therapy, authorized physician or authorized podiatrist, including directions as to treatment, if any, shall constitute a part of the record of the case.
