Section 326-1.2 - Application for license to operate medical bureau or laboratory

Applications for licenses to operate compensation medical bureaus or laboratories and for other laboratories and bureaus shall be made upon the application form prescribed for the purpose by the chair and shall be filed with the county medical society, or the workers' compensation committee of the county medical society, of the county in which such medical bureau or laboratory is located.
