Section 325-2.5 - Waiver agreements related to medical benefits or treatment

No waiver agreements with respect to an injured employee' s medical benefits or treatment beyond the properly executed prescribed consent form shall be valid and enforceable unless reviewed and approved by the board in accordance with section 32 of the Workers' Compensation Law and section 300.36 of this Title.
