Section 175.4 - Driver to fill out daily time record

A driver of a motor truck and/or motor bus shall daily fill out and keep the daily time record form:

(a) Immediately upon going on duty such driver shall fill in on the daily time record the date, vehicle license number, name of carrier, carrier's main office and terminal address, and the time and starting point or place the driver goes on duty.
(b) Thereafter, the driver shall show on the daily time record the actual period or periods during the calendar day spent in the activities specified on lines 1 (off-duty), 2 (sleeper berth), 3 (driving) and 4 (on duty--not driving), by drawing a continuous line between the appropriate time markers. Immediately before each change in duty status, he shall continue the existing continuous line by drawing a vertical line into the area of the new duty status.
(c) When his day's duty is completed, the driver shall fill out all other information required in the form, and shall sign his name in full in the appropriate place to certify that the entries are true and correct. The employer or his responsible agent shall then indicate his knowledge that this has been done by signing his name on the form in the appropriate place.
