Section 128-1.5 - Local representatives

(a) Information about these rules and regulations and application and other forms required by these rules and regulations may be obtained from the following offices of the department. Applications for department approval of a regulated activity must be submitted to one of the offices of the department listed in subdivision (b) of this section. Petitions appealing from a determination issued by the department or requesting a hearing on a cease and desist order issued by the department must be submitted to the offices listed in subdivision (c) of this section.
