Section 16.23.1.15 - DUPLICATE LICENSE In the event a license or permit is lost or destroyed, the department will issue a duplicate license or permit upon receipt of the following.
A. Notice to the department of the loss by the licensee or permitee.B. A request for a duplicate.N.M. Admin. Code § 16.23.1.15
