Section 8:43J-2.9 - Surrender of license(a) The facility shall notify the Department, each child's parent, the child's primary health care provider and any guarantors of payment: 1. At least 30 days prior to the voluntary surrender of a license or relocation of a facility; or2. As the Department directs in an order of revocation or suspension or in a notice of refusal to renew.(b) The facility shall return the license to the Office of Certificate of Need and Healthcare Facility Licensure within seven working days after the voluntary surrender or the revocation, suspension or non-renewal of its license.
