Section 8:34-6.6 - Change of name, address and/or employment

(a) A licensed administrator shall notify the Board in writing within 15 calendar days of a change in his or her name and shall include with the notice written and signed documentation of the change of name and a certified copy of the court order or marriage certificate, as applicable.

1. Upon the Board's receipt of the notice described in (a) above, the Department shall issue to the licensee a replacement license reflecting the name change, upon receipt of which the licensee shall return the original license to the Board.

(b) A licensed administrator shall notify the Board in writing within 15 calendar days of a change in his or her home address.

(c) A licensed administrator shall notify the Board in writing within 15 calendar days of any change in his or her place of employment.

(d) A licensed administrator shall retain a copy of written notification to the Board of any change in his or her name, address and/or employment.
