Section 8:43G-11A.8 - Discharge

(a) The discharge planning team shall notify the caregiver in advance of the patient's discharge or transfer to another facility.

(b) The discharge planning team shall document promptly in the patient's medical record all attempts to contact the caregiver.

1. If required, the discharge planning team shall make multiple attempts to contact the caregiver.

(c) The inability of the discharge planning team to contact a caregiver shall not delay a patient's discharge or transfer.
