A claimant shall designate a person as the proper addressee of communications from the Department under this subchapter. To designate such a person, the claimant shall submit the following information to the Department in writing: the name and address of the claimant; the name, address, and telephone number of the designated person; and a request that all Department inquiries and communications (oral and written), including, without limitation, the inquiries and notices listed in 7:26-17.5(a), be directed to the designee.
