Section 14:18-6.3 - Accidents or injury to personnel

Each cable television company shall keep a record of, and report to the Office, any serious accident or any serious injury to or death of personnel involved in the operation or maintenance of its system or any serious accident resulting from the operation or maintenance of its system on forms prescribed and furnished by the Office in addition to providing a detailed narrative explaining the cause of the accident, any corrective action taken, as well as copies of any police reports generated as a result of the accident.
