N.J. Admin. Code § 13:39-3.3

If a pharmacist legally changes the name under which he or she engages in the practice of pharmacy, the pharmacist shall notify the Board within 30 days of such change. The pharmacist shall submit original proof of the change of name or a certified copy of the court order or marriage certificate, which shall be retained by the Board. When a replacement license is issued, the initial license shall be returned for cancellation and the pharmacist shall remit the required fee as prescribed in 13:39-1.3.
