Section 13:103-5.3 - Transportation documentation(a) Prior to accepting escort responsibility for any juvenile, escorting staff members shall be informed of the following: juvenile's name and number; destination; supervision status; and unusual medical, emotional, or mental conditions for which there is a need to know. Escorting staff members shall also be provided appropriate medical information setting forth any prescribed medication or instructions for special handling when transporting juveniles with medical or psychiatric conditions.1. If a required medical report is not available, the escort staff members shall request the supervisor on duty at the sending community program to provide a brief written statement that the injury or condition existed prior to turning over the custody of the juvenile.(b) Prior to escort to court and on inter-facility transfers, the escorting staff members shall ensure that they have all documentation necessary to obtain custody and/or to effect delivery of the juvenile being escorted.(c) Staff members escorting a juvenile shall ensure that any trip authorization or other Commission required travel documents are properly completed and returned to the community program upon completion of the trip.(d) All escorting staff members shall carry their Commission photo identification card.
