Section 10:37-6.108 - Unusual incident reporting system
(a) Every Agency funded to provide services in a Mental Health Program Element shall submit reports, as required by the Division regarding incidents that may, or already have, adversely affect(ed) the health or safety of client(s). An explicit description of the types of incidents that must be reported and an appropriate reporting format shall be developed by the Division and disseminated to all participating agencies.
(b) Any incident that includes an allegation of child abuse and/or neglect must be immediately reported to the Division of Youth and Family Services (DYFS), in compliance with N.J.S.A. 9:6-8.1 0. Abuse or exploitation in rooming/boarding/nursing homes shall be reported to the County Welfare Agency.
(c) This section does not, however, negate the necessity of making entries into a client's clinical record concerning accidents and injuries. (See 10:37-6.74(a)11.)
