Section Saf-C 804.01 - Application, Data Required(a) Any person applying for permission to connect an alarm shall furnish to the receiving agency the following:(1) The name, address, and telephone number of the subscriber;(2) The type of protection afforded by the alarm, whether burglary or fire;(3) The name, address, and telephone number of 2 persons to be called in the event of alarm activation;(4) Whether the alarm is monitored by any other person or agency, and if so, the name, address, and telephone number of same;(5) The name, address, and telephone number of the alarm service company;(6) The subscriber's signature; and(7) The approving officer's signature.(b) The information above shall be provided on a form prescribed by the receiving agency.
