Section Saf-C 6510.03 - Content of Monitoring Reports

Monitoring reports shall include at minimum:

(a) A statement that the device:
(1) Is calibrated;
(2) Has been inspected and is functioning properly; and
(3) Shows no evidence of tampering; and
(b) The, date, time, and location of the next monitoring check;
(c) The name, location, and telephone number of the interlock service provider;
(d) Photos supporting each breath test or attempt within the report;
(e) The date and time of each event listed;
(f) The name, date of birth, and address of the user;
(g) Driver license number and driver license state of the user;
(h) The following vehicle information:
(1) Make, model, and year;
(2) Vehicle identification number;
(3) License plate number and state of issuance; and
(4) Odometer reading; and
(i) Calibration readings of the device;
(j) Make, model, and serial number of the device;
(k) Dates of the monitoring period;
(l) Any comments from the interlock service provider; and
(m) Jurisdiction(s) where the user is being monitored.
