Section Saf-C 4603.03 - Bills(a) Bills submitted for services rendered shall contain the following:(1) The name and address of the carrier;(2) The name and address of the shipper;(3) The date or dates the service was performed;(4) The points between which the service was rendered; and(5) Each item for which a rate is applied, as well as the corresponding rate applied.N.H.
