Section Saf-C 414.02 - Water Event or Slalom Course Permit Application(a) An applicant for a permit to conduct a water event or slalom course shall provide the commissioner with the following: (1) Name, address, date of birth, telephone number and cellular telephone number, if available, of the applicant;(2) The type of water event;(3) The name of the water event, if applicable;(4) The name of the public water on which the water event will be conducted;(5) The name of the town or city where the public water is located;(6) The date(s) the water event will be conducted;(7) The start and end time of the water event each day it is conducted;(8) A brief description of the water event;(9) The number of participants in the water event;(10) A map or sketch of the water event area, including the direction "north" as a reference; and(11) Signature of the applicant and date signed.N.H.
