N.H. Admin. Code § Saf-C 3119.01

Section Saf-C 3119.01 - Application Forms
(a) Whenever a person is required to complete a form in parts Saf-C 3118 through Saf-C 3123, the following shall apply:
(1) Forms shall be filled out completely; and
(2) Forms shall be typewritten or completed in a legible manner.
(b) Application forms may be obtained from:

Department of Safety

Division of Motor vehicles

Driver Education Section

23 Hazen Drive

Concord, NH 03305.

(c) A completed application shall include the following:
(1) Proof of driving experience as set forth in Saf-C 3118.02;
(2) Report of physical examination as set forth in Saf-C 3118.03;
(3) Successful completion of written and on-road examinations as set forth in Saf-C 3118.04;
(4) Criminal history record check;
(5) Fingerprints upon initial application only, pursuant to RSA 263:44-b; and
(6) Motor vehicle record from any state the applicant has held a driver license.
(d) Completed application forms shall be submitted to the:

Driver Education Section

(e) All forms shall be reviewed by the division in accordance with RSA 541-A:29.
(f) Applicants shall be notified of any apparent errors or omissions and allowed to resubmit the form to the division.
(g) In the event an applicant fails to comply with an applicable statute or these rules, the application shall not be processed and the applicant shall be notified of the reason(s) for the rejection.
