Section Saf-C 3108.02 - Approved Drivers' School Application(a) Each applicant for an approved drivers' school license shall complete and submit form DSMV 604, "Drivers' School Application" (rev 09/17).(b) Each applicant, including individuals, shall certify that all vehicles which will be used by the drivers' school comply with all applicable laws and rules.(c) Each applicant, including individuals, partners, and officers shall provide a certified copy of his/her motor vehicle record from any state in which the applicant has held a driver license and from any other state in which the applicant has obtained a motor vehicle conviction.(d) Each applicant, partners, and officers shall provide a certified copy of his/her criminal record from New Hampshire and from any other state in which the applicant has obtained a criminal conviction.(e) Each applicant shall sign and date the form under the penalty of unsworn falsification.N.H.
