Section Saf-C 311.45 - Form RT 134a, Biodiesel Distributor License Certificate(a) The department's form RT 134a shall contain the following: (1) Company or person's name and address;(2) Federal employer identification number, if applicable;(3) Statement of official licensing;(4) Date of issuance; and(b) The commissioner or his/her designee shall sign the form.N.H.
