Section Saf-C 311.38 - Form RT 129, Application for IFTA

(a) An applicant shall submit the following on form RT 129:

(1) Applicant's name and address;

(2) Federal employer identification number, if applicable;

(3) Fuel type;

(4) Gross weight;

(5) Location of records for audit purposes;

(6) Type of entity;

(7) Principal type of business; and

(8) All required fields.
