(a) An applicant shall submit the following on form BDF 1: (2) Licensee's: d. Federal employer identification number, if applicable;(3) Opening inventory, which must agree with the prior month's ending inventory and includes items in transit;(4) Total receipts from schedules 2, 2E, 2F, 3, and 4;(5) Gallons of biodiesel refined, distilled, blended or manufactured;(6) Total disbursements from schedules 5, 6, 6F, 7, 8, and 9;(8) Ending physical inventory;(10) Total of tax-paid credits from schedules 13C and 13H;(11) Net taxable gallons;(12) Tax rate per gallon;(b) The preparer shall print his/her name and title, sign and date the form.N.H.
