Section Saf-C 2105.01 - Form of Request

Each request for a hearing on a suspended or revoked license shall:

(a) Be in writing from the person requesting the hearing; and
(b) Shall contain the following information:
(1) Name;
(2) Address;
(3) Telephone number; and
(4) The reason(s) for contesting the suspension or revocation.
