Section Saf-C 1902.09 - Change of Name(a) In the event a person changes his or her name, he or she shall furnish the following on form DSMV 30 "Record Change Request" (rev. 3/22):(b) As evidence of a new name, a person shall present a copy of one of the following:(1) Name change petition from the court of probate;(2) Marriage certificate;(c) In the event a corporation, partnership, or other legal entity changes its name, it shall be presumed that the change constitutes a transfer of ownership, unless satisfactory proof to the contrary is furnished, such as a certificate from the secretary of state's office showing the name was changed and that no corporate officers have changed.N.H.
