(b) Each applicant shall complete and submit form DSMV 312, in person to a division substation.(c) Applicants shall submit form DSMV 312 to a division substation for each of the following: (1) Original commercial driver license;(2) Renewal commercial driver license;(3) Duplicate commercial driver license; and(4) Replacement commercial driver license.N.H.
