Section Saf-C 1607.04 - Notifications(a) A licensee shall promptly notify the director of a change in business address.(b) A licensee shall report to the director, within 24 hours of the discovery of any loss, theft, or unlawful removal of explosive materials.(c) At least 24 hours prior to blasting, the individual who is responsible for the overall blasting operation shall notify the local police chief and local fire chief of the location of the blast area.(d) A licensee, prior to the placement of a magazine, shall notify the director and the local fire chief of the location of each unattended overnight storage facility and the estimated length of time the facility will be at the location. The director and the local fire chief shall also be notified promptly upon removal of the facility at the end of its use at the location.(e) A licensee shall immediately notify the director if there is any explosion, fire or other accident occurring in connection with the keeping, storage, manufacture, handling, transportation, use or other disposition of explosives causing loss of life or injury to any person or damage to property in excess of $200.00.(f) The notification shall: (1) Be made by telephone at (603) 271-3575;(2) Contain an account of the accident; and(3) Be confirmed in writing pursuant to (h) below.(g) A written accident report shall be mailed by the licensee.(h) The licensee shall submit the following on the accident report: (1) The date of the report;(2) Certificate of competency number of the person conducting the blasting operation;(3) The name and address of the person conducting the blasting operation;(4) The time of accident;(5) The exact location of the accident;(6) The name and address of any person(s) injured or killed;(7) The weather conditions at the time of the accident;(8) The type and amount of explosive;(9) A description of the accident and total cost of repair;(10) The cause of the accident, if known;(11) The names and addresses of any witnesses, if known;(12) The name of the hospital or doctor to which or whom the injured person(s) was taken;(13) The type of first aid equipment that was available at the scene of the accident;(14) A detailed description of the accident; and(15) The signature of the licensee or individual who conducted the blasting operation.N.H.
