Section Saf-C 1204.06 - Required Notifications

(a) A holder of an individual certificate shall notify the department, in writing, by mail, to the address set forth in Saf-C 1204.03(a), within 30 calendar days of the following:

(1) A change in name or address; or

(2) A change in employer.
