Section Saf-C 1016.05 - Parent or Guardian Authorization Certificate(a) If the department receives written information which indicates the death of a signatory of a "parent or guardian authorization certificate," form DSMV 38 as set forth in Saf-C 1002.04, the financial responsibility administrator shall suspend the named minor's license.(b) If the signatory of a "parent or guardian authorization certificate" withdraws such consent, the or financial responsibility administrator shall suspend the named minor's license. Such withdrawal of consent shall be in written form.(c) The withdrawal of consent, pursuant to (b) above, shall include:(1) The withdrawal of consent;(2) The name of the parent or guardian; and(3) The name of the minor.(d) As long as all restoration fees have been paid, suspension under this section shall remain in effect until the earlier of: (1) The named minor attains the age of 18;(2) A new "parent or guardian authorization certificate" is received;(3) Proof of insurance in the form of an SR-22 certificate covering the named minor is received; or(4) The person under 18 years of age is emancipated by marriage.N.H.
