Section Saf-C 3104.06 - Application for Renewal of Driver Education Standard Certificate(a) Each applicant for the renewal of a driver education standard certificate shall complete and submit the form DSMV 635, "Application for Driver Education Instructor Certificate" (rev 02/18).(b) The applicant shall provide a certified copy of his/her motor vehicle record from any state in which the applicant has held a driver license and from any other state in which the applicant has obtained a motor vehicle conviction.(c) The applicant shall provide a certified copy of his/her criminal record from this state and from any other state in which the applicant has obtained a criminal conviction.(d) The applicant shall sign and date the form under penalty of unsworn falsification.(e) Pursuant to RSA 21-P:14, I(f), a fee of $40 shall be submitted along with the application.N.H.
