Section Saf-C 311.46 - Form RT 134b, Alternative Fuel Dealer License Certificate

(a) The department's form RT 134b shall contain the following:

(1) Company or person's name and address;

(2) Federal employer identification number;

(3) Statement of official licensing;

(4) Date of issuance; and

(5) Certificate number.

(b) The commissioner or his/her designee shall sign the form.
