Section Saf-C 311.37 - Waiver of Statute of Limitations Form(a) An applicant shall submit the following on the waiver of statute of limitations form: (1) Certification that the authorized representative can sign the form on behalf of the company or entity;(2) The entity's name and federal employer identification number, if applicable; and(3) Preparer's name and title.(b) The applicant shall sign, date and have the form notarized.N.H.
