Section Saf-C 311.06 - Form BDF 1 Biodiesel Fuel Distributor Report

(a) An applicant shall submit the following on form BDF 1:

(1) Period covered;

(2) Licensee's:

a. Name and address;

b. Telephone number;

c. License number; and

d. Federal employer identification number, if applicable;

(3) Opening inventory, which must agree with the prior month's ending inventory and includes items in transit;

(4) Total receipts from schedules 2, 2E, 2F, 3, and 4;

(5) Gallons of biodiesel refined, distilled, blended or manufactured;

(6) Total disbursements from schedules 5, 6, 6F, 7, 8, and 9;

(7) Gain or loss;

(8) Ending physical inventory;

(9) Taxable gallons;

(10) Total of tax-paid credits from schedules 13C and 13H;

(11) Net taxable gallons;

(12) Tax rate per gallon;

(13) Tax due; and

(14) Total amount due.

(b) The preparer shall print his/her name and title, sign and date the form.
