Section Saf-C 1204.05 - Issuance of Individual Certificate

(a) Each individual certificate issued shall contain the following:

(1) Name and address of individual;

(2) Certificate number;

(3) Any endorsements;

(4) Date of expiration; and

(5) Signature of the state fire marshal.

(b) An individual certificate issued shall expire on the last day of the individual's birth month, 3 years from the date of issuance.
