Section Res 2606.02 - Requirements to Request Grant Amendment(a) Each business requesting a grant amendment shall provide the following information: (1) Name, address and telephone number of the business;(2) Name and title of the contact person as required in Res 2604.01(a)(2);(3) Approved grant number, issued by the grant administrator;(4) Detailed description of the requested grant amendment, including: a. Original grant amount;c. Grant amount difference; and(5) A statement detailing why the commissioner should approve this amendment.(b) The request shall be addressed to the commissioner.(c) All information shall be typewritten.(d) The contact person representing the business shall sign and date the request.(e) The request shall be mailed or hand-delivered to: Department of Resources and Economic Development
