Section Puc 1107.07 - E-23 Report of Interruptions of Service

(a) Each utility shall file Form E-23 monthly with the commission which shall report interruptions of service.

(b) Utilities shall include on form E-23 the following:

(1) Date;

(2) Time of interruption including start, end and total elapsed time;

(3) Location;

(4) Number of customers; and

(5) Cause of interruption.
