Section Plc 708.11 - Information Required for School Approval Renewal Application The applicant for renewal of an electrolysis school approval shall provide the following information:
(a) The owner's name, address, telephone number, and the email address to which notices and approvals for the school can be sent by the OPLC, hereinafter called the "designated email address";(b) The name, address, and telephone number of the school, and, if available, the URL of the school's website and the school's email address;(c) The maximum number of electrology students the school intends to instruct at any one time; (d) If the owner is a business entity, the name and title of the individual who has been duly authorized to sign the application and otherwise interact with the OPLC regarding the application, along with the authorized individual's telephone number and email address;(e) For each electrology instructor, the individual's name, email address, license number, number of years of experience, and CPE credentials to show compliance with Plc 708.02(f);(f) A list of all electrology and sterilization equipment the school uses, and for each the type of equipment, the model if applicable, the manufacturer, and whether the equipment is ANSI certified or FDA approved;(g) Confirmation that the school meets the requirements of Plc 707.01.N.H. Admin. Code § Plc 708.11
