"Treatment/monitoring plan (T/M plan)" means a specific course of action tailored to the specific needs and circumstances of a participant who has been identified as having a treatable or modifiable condition, which may include education, intervention, ongoing care or treatment, or post-treatment monitoring, or any combination thereof. A T/M plan is a component of a program agreement for participants having a treatable or modifiable condition.
