Section Plc 305.03 - Applications for Temporary License(a) An applicant for a temporary license shall submit: (1) A completed application for initial licensure as provided in Plc 304, which may be submitted prior to or with the application for temporary license;(2) The information specified in Plc 305.04(a)-(k) on a "Universal Temporary License Application" dated March 2023, that is signed and attested to as specified in Plc 305.05;(3) The verification specified in Plc 305.04(l); and(4) The temporary license application fee specified in Plc 1001.(b) The applicant shall submit the required items to the licensing bureau:(2) If the portal is not yet available, by mailing or delivering to the OPLC office at the address listed in Plc 102.03.N.H.
